New Patient Information
Before scheduling your first visit, please complete the New Patient Information Form, accessible by selecting and downloading either the digital format* or handwritten format. Please read the instructions carefully. Receiving your paperwork in advance allows us to thoroughly review your information prior to your initial visit. You may email (info@drpeine.com), mail, fax (208-947-0926), or drop off the completed forms at our office. It is our office policy that you must return your completed and signed New Patient Information Form to our office before we will schedule your first appointment. Upon receiving your paperwork, we will call to schedule your appointment. If you cancel your first New Patient appointment, all of your follow-up appointments will automatically be cancelled. Our office schedule is often full several weeks to months in advance. As a courtesy to other patients, please provide at least 24 hours notice if you are unable to make your appointment. There is a $100 fee for cancellations less than 24 hours in advance or no-shows.
